Who Qualifies for Hospice Care?
Patients qualify for hospice care when a physician has certified that their illness is terminal with a life expectancy of six months or less if the disease runs its natural course. All care for the hospice patient is provided under the orders of a physician. Diagnoses commonly associated with hospice care may include, but are not limited to, any of the following illnesses:

Who Is on Your Support Team?
Charles Ford Hospice in Harmony provides a multitude of individuals and disciplines to assist you and your family. Our care team consists of skilled healthcare professionals whose roles often overlap.
- Hospice Medical Director, who works in collaboration with your attending physician
- Skilled nursing professionals, including Registered Nurses (RN) and Licensed Practical Nurses (LPN)
