Who Qualifies for Hospice Care?

Patients qualify for hospice care when a physician has certified that their illness is terminal with a life expectancy of six months or less if the disease runs its natural course. All care for the hospice patient is provided under the orders of a physician. Diagnoses commonly associated with hospice care may include, but are not limited to, any of the following illnesses:

Who Is on Your Support Team?

Charles Ford Hospice in Harmony provides a multitude of individuals and disciplines to assist you and your family. Our care team consists of skilled healthcare professionals whose roles often overlap.

  • Hospice Medical Director, who works in collaboration with your attending physician
  • Skilled nursing professionals, including Registered Nurses (RN) and Licensed Practical Nurses (LPN)
  • Certified Nursing Assistants (CNA)
  • Licensed social worker
  • Chaplain
  • Bereavement counselor
  • Trained volunteers
  • Physical, occupational, and speech therapists (as needed)

Our hospice team is available 24 hours a day, 365 days a year.

Who Pays for Hospice Care?

Hospice care is covered by either Medicare, Medicaid, or commercial insurance. The following services are included in your benefit coverage:

  • All staff visits
  • Medication related to terminal illness
  • Durable medical equipment (DME) related to terminal illness
  • Nutritional, incontinence, and wound supplies

There are four different levels of hospice care:

  • Routine home care
  • Respite care
  • Continuous home care
  • General inpatient care
